THE ORIGINAL APPLICATION MUST BE SUBMITTED IN DOUBLE-SIDED FORMAT.
The application MUST BE SIGNED AND PROPERLY NOTARIZED and filed in duplicate,
the original plus a good photocopy.
"Original" means a double-sided copy bearing the applicant's
original handwritten signature plus the original notarization. The Notary must affix the PROPER SEAL OR
STAMP as well as his or her original signature on all documents requiring
notarization. REPRODUCED COPIES OF THE
APPLICATION FORM NOT BEARING THE ORIGINAL HANDWRITTEN SIGNATURE AND ORIGINAL
NOTARIZATION WILL NOT BE ACCEPTED AS THE ORIGINAL. Attachments do not have to be filed in
duplicate.
PLEASE NOTE that common ERRORS THAT WILL CAUSE AN
APPLICATION TO BE REJECTED include (but are not limited to) failure to sign the
application, failure to have the signature properly notarized, failure to sign
the authorization and release forms, and failure to include proper
payment. These errors will not
necessarily be detected by the Board's staff until after the filing deadline
has passed, even if an application is filed early. It is the applicant's responsibility to
ensure that the application is complete and in proper form prior to the
deadline. Late detection of errors by
the Board will NOT extend the filing deadlines.
